Community Learning Disability Team

The Community Learning Disability team are commissioned to provide care, treatment and support to people with learning disabilities.

The team who work with people with a learning disability have specialist skills and work in a multi-disciplinary way alongside a range of health professionals. They provide an assessment service and work with people and their carers or families to develop a plan of targeted interventions to ensure the safety, health and wellbeing of the person.

The team aims to support people with a learning disability by supporting them to access facilities in their local communities.
