Active Recovery Team
Active Recovery is used to work with people over a defined period of time to achieve goals with the overall aim of maximising their independence, health and quality of life. It is aimed at adults who have a new or longer term need for services but could be supported to recover or minimise their needs, reducing the need for longer term care.
The Active Recovery Assessment team carry out the assessments and reviews to support people to identify specific goals and tasks to aim for during their period of recovery. Equipment Assessors work in this team to identify which specific equipment can be provided to support people towards improved health, wellbeing and independence.
