Hospital Patient Passport’s

CHCP’s Primary Care Liaison Service support people with learning disabilities, autism and severe mental illness to access GP practices and appointments. They also look at the uptake of the annual health checks for these people and ultimately, their key focus is reducing the health inequalities of these people.
