Connect Well Hull

Connect Well Hull is a free and confidential service for people who live in Hull or are registered with a Hull GP. The service can help you try something new, get active, or speak to someone about benefits, housing or money worries.

You can book a face-to-face appointment with a Wellbeing Co-ordinator or Welfare Adviser at a range of locations across the city. You can make an appointment by using the link below or telephone them on 01482 217 670.

Adult social care self assessment

If you have not found what you need on the Connect to Support website, or Connect Well Hull is not for you, you may want to complete an adult social care self assessment.

You can complete a self assessment if you are 18 or over and feel that you need that extra support to live your life independently. The self assessment will help you think about what works well in your life and what needs to change. It may be helpful to do this with a family member, friend or someone else you trust.
