What is the ICC
The Jean Bishop Integrated Care Centre (ICC) is the UK’s first ever centre that combines health and social care support for older people as an alternative of going to the hospital.
The Centre is a one stop shop for a range of support services and has a comprehensive specialist team including -
- Older People’s Consultants/Geriatricians
- Advance Nurse Practitioners
- Social Workers
- Voluntary and community sector organisations.
There are diagnostics at the centre enabling the team to undertake blood tests, x-rays and CT scans.
How the referral process works
Your GP will look at your overall health and wellbeing and assess whether would benefit from accessing the ICC’s services.
Once your GP makes a referral, you will receive a home visit from one of the ICC’s team members. They will undertake an assessment and identify issues that you wish to discuss when you attend the ICC.
You will then receive an appointment time to visit the ICC. Once you are at your appointment, you will have access to a wide range of support services to help you to stay independent.
You will then receive a care plan which you will be able to take home and follow.
Benefits to the ICC
Attending the ICC is an alternative approach to accessing primary healthcare services and can reduce the amount of time you need to spend on appointments at the hospital.
It can also help reduce the risk of accidents that require hospital treatment by helping you to find solutions to help you to stay safe and independent in your own home.
